RELIA HOMECARE LLC – TERMS & CONDITIONS

These Terms & Conditions (“Terms”) govern the use of the website https://reliahomecare.com and the services provided by Relia Homecare (“Relia,” “we,” “us,” or “our”).
By accessing our website, contacting us, or receiving services from Relia Homecare, you agree to these Terms. If you do not agree, do not use our website or services.
1. SERVICES PROVIDED
Relia Homecare provides in-homecare services based on a written Service Agreement and individualized Care Plan.
Services may include, but are not limited to:
NON-MEDICAL / SUPPORTIVE CARE
– Personal Care (bathing, grooming, dressing, toileting assistance)
– Companion and Sitter Care
– Respite Care for family caregivers
– Meal preparation and light housekeeping related to care
– Medication reminders (non-clinical)
– Mobility and transfer assistance
SKILLED NURSING SERVICES
– Skilled nursing services performed by appropriately licensed nurses when ordered, authorized, and permitted by law and the Care Plan.
All services are subject to availability, staffing, safety considerations, and applicable laws.
2. LICENSING AND COMPLIANCE
Relia Homecare operates in accordance with applicable Georgia state laws and regulations, including licensing requirements for private homecare and skilled nursing services.
3. NOT EMERGENCY CARE
Relia Homecare does NOT provide emergency medical services.
If a medical emergency occurs, CALL 911 immediately.
4. CARE PLAN AND ELIGIBILITY
Services begin only after:
– An intake assessment
– Completion of a Service Agreement
– Agreement on scope, schedule, and rates
Relia may refuse, suspend, or discontinue services if:
– Client needs exceed what can be safely provided
– The home environment is unsafe
– Required information is not provided
– Abuse, harassment, or unsafe behavior occurs
– Payment or insurance authorization issues arise
5. CLIENT RESPONSIBILITIES AND HOME SAFETY
Clients and families agree to:
– Provide accurate health, insurance, and emergency contact information
– Maintain a safe home environment for caregivers
– Secure pets when requested
– Disclose known safety risks (falls, aggressive behavior, infections, weapons)
– Provide required supplies or equipment if needed for care
Relia staff may end a visit if conditions are unsafe.
6. SCHEDULING AND CAREGIVER SUBSTITUTION
– Visit times are approximate and may vary due to traffic, weather, or prior visits
– Relia may substitute caregivers or nurses as needed
– A specific caregiver is not guaranteed for every visit
If no one answers the door, refuses entry, or refuses services after arrival, the visit may be billed according to the cancellation policy.
7. SKILLED NURSING LIMITATIONS
– Skilled nursing services are provided only when clinically appropriate and authorized
– Relia staff do not diagnose conditions or replace a physician
– Tasks outside the Care Plan or legal scope may be declined
8. CANCELLATION POLICY (24 HOURS)
Clients must cancel at least 24 HOURS before the scheduled start time.
Cancellations with less than 24 hours’ notice, or inability to provide care due to access refusal or unsafe conditions, may result in charges including:
– Full scheduled visit hours, or
– A late-cancellation fee as outlined in the Service Agreement
Repeated late cancellations may result in schedule changes or service discontinuation.
9. INSURANCE AND THIRD-PARTY PAYERS
Relia Homecare accepts certain insurance plans and third-party payers.
IMPORTANT:
– Insurance coverage is NOT a guarantee of payment
– Prior authorization, eligibility, and medical necessity rules apply
– Denied or reduced claims are the client’s responsibility unless prohibited by law
Clients agree to provide accurate insurance information and cooperate with billing and documentation requirements.
10. PRIVATE PAY SERVICES
For private-pay services:
– Rates and payment terms are outlined in the Service Agreement
– Late payments may result in late fees or service suspension
11. MEDICATION REMINDERS
Medication reminders are non-clinical.
Relia does not prescribe, administer, or change medication instructions.
For adverse reactions or emergencies, contact the prescribing provider or call 911.
12. PRIVACY AND HEALTH INFORMATION
Relia Homecare respects client privacy and safeguards personal and health information.
Information may be shared as needed for:
– Care coordination
– Billing and insurance
– Communication with authorized family members or providers
Clients consent to such use consistent with applicable privacy laws and written permissions.

14. COMMUNICATION CONSENT
By providing contact information, you consent to receive calls, texts, or emails related to scheduling, care coordination, documentation, and billing.

15. INTELLECTUAL PROPERTY

16. THIRD-PARTY LINKS
Relia is not responsible for third-party websites, content, or services linked from our website.
17. LIMITATION OF LIABILITY
To the fullest extent permitted by law, Relia Homecare is not liable for indirect, incidental, or consequential damages.
Total liability shall not exceed the amount paid for the specific service giving rise to the claim, except where prohibited by law.
18. GOVERNING LAW AND JURISDICTION
These Terms are governed by the laws of the State of Georgia.
Any disputes shall be resolved in Georgia state or federal courts.
19. CHANGES TO TERMS
Relia Homecare may update these Terms at any time.
Continued use of services or the website constitutes acceptance of updated Terms.
